At Senior Solutions (Area Agency on Aging in Southeastern Vermont) we are seeking aCase Managerto work with older adults and/or people with disabilities inWindsorVermont and the surrounding areasusing a person-centered planning approach and a deep understanding of state and federal benefits. Strong organizational communication and computer skills are required.
Qualifications:
Bachelors degree in human services and prior case management experience preferred.
A combination of relevant education and experience may be substituted for a degree.

We are always looking for team members who are driven to quickly learn new skills about all aspects of the benefits and services available to aging Vermonters. Must have reliable transportation and a valid drivers license.
Responsibilities include:
- Collaborating with other community partners to create a person-centered plan of care.
- Monitoring and reassessing program service plans as required.
- Having a working knowledge of services and programs available to older Vermonters and people with disabilities.
- Assisting when necessary to gain access to existing services such as public benefits housing legal services etc.
- Ensuring that all provided services do not exceed budgeted amounts and are consistent with DAIL guidelines.
