Requirements

Applicants must provide the following:

Level 1 Fingerprint Clearance Card
CPR/BLS certification from the American Heart Association
Current TB skin test
Copy of current valid unrestricted CNA license issued by the Arizona State Board of Nursing
- OR issued by a participating Compact State
COVID-19 vaccination or signed declination of offered vaccine
