Job Summary

The Health and Welfare Council of Long Island (HWCLI) is a private not for profit health and human services planning research/public education and advocacy organization that serves as the umbrella for public and non-profit agencies serving Long Islands poor and vulnerable individuals and families.

The Health Equity Alliance of Long Island (HEALI) is Long Islands Social Care Network (SCN) of community-based organizations (CBOs) and healthcare providers building a person centric model that integrates healthcare social care and behavioral health care. HWCLI is theleadentity for the HEALI Social Care SCN brings together social service and health care providers from across Nassau and Suffolk counties to provide enhanced healthcare equity through improvements in need identification care coordination integration and provision of tailored funding.

JOB ANNOUNCEMENT:Technical AssistanceSpecialist Health Equity Alliance of Long Island

HWCLIseeksan experienced energetic passionate and socially conscious individual to support the HWCLI and HEALI missions by providing client-centeredtechnicalsupportand training. This roleis responsible forensuring successful participation and performance of network organizations throughtraining deliveryand targeted technicalassistanceprovision. This role willfocus on day-to-day operations of the training delivery system training development processes andprovide collaborative coaching training and quality improvement activities to ensure successful performanceacross the Network. This position reports to theService Design Program Director.

Responsibilities include:
  • Serves as a regional expert ofall SCN functionsand providesin-person and virtual technicalassistanceand trainingsupport to optimize service integration and delivery across a network of CBOs;
  • Coordinatethe design and delivery oftargeted training and TA interventions in response to performance trends;
  • Manage the day-to-day functions of the network Learning Management System to ensure training completion meets network standards
  • Identifythe need fortraining and resourcematerials and tools ( guides reference documents tip sheetsetc).Develop and deploy and connect partners toappropriate resourcesandstakeholders;
  • Provide technicalassistanceto all onboarded network partners through ongoing assessment and evaluation training policy and procedure optimization and implementationstrategy;
  • Meet withpartner organizationsas needed tosupport onboarding and ongoing challengesensure information in the network is still up to date review networkutilization compliance with network standards successes and challenges
  • Directly support local CBOs to buildcapacitywithHRSN screening/navigation/HRSN case management/service deliverycapabilities;
  • Contribute to the development of routine performance reports both internally andexternally;
  • Collaborate closely with Unite Us support team to developappropriate recommendationsfor system and workflowenhancements;
  • Other responsibilities asneeded

Qualifications and Experience:

  • Bachelors degreerequired
  • Valid NYS drivers license and the ability to travel locally in Long Island as needed for in-person meetings
  • 1-3 years of project management capacity buildingtraining deliverytechnicalassistance or quality improvement experiencepreferred
  • Experience working with Unite Us platform or similar referral and care management tools strongly preferred
  • Prior experience in issues related to healthcare; social services; health equityaccessand quality; population health (or some subset/combination of these) preferred.
