Job Summary and Responsibilities
As our RN you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management.
Every day you will perform a wide variety of high-level patient care activities to include admission assessment screen for appropriateness and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patients post-discharge needs and the provision of services necessary to meet those needs.
To be successful in this role you must be knowledgeable in community resources and alternate care facilities.
- PATIENT ASSESSMENT/REASSESSMENT
- Perform patient assessment/reassessment including giving or receiving comprehensive change of shift and intermittent report obtaining the nursing history conducting a physical assessment establishing priorities for care.
- Assess patients learning needs readiness for discharge and ability of the patient/family/SO to manage illness after discharge.
- Completes both computer and hand written documentation using correct format in an accurate precise timely legible and complete manner .
- Provide direct patient care as required. Assist physician with procedures as necessary. Is competent in IV Therapy.
- Report changes in patients condition to physician.
Job Requirements
Required
- High School Graduate Education level necessary to obtain required licensure and certifications.
- 1-2 years of previous experience.
- Registered Nurse: CA required
- Basic Life Support - AHA required
- Advanced Cardiac Life Support AHA required
Preferred
- Bachelors Of Nursing preferred
