Job Description:

Organizational Responsibilities

  • Knowledgeable of Operations Manual (Policy & Procedure)
  • Acceptance of the Salvation Army mission statement
  • Acceptance of the Salvation Army code of ethics
  • Ensure that all residents abide by conditions of Parole and house rules
  • Engage in regular evaluations with the Program Coordinator
  • Caseload 40-60% of Railton residents (21 beds) or as agreed with the Program Coordinator manage caseload lead case meetings and provide required reports for allocated residents
  • Represent Railton House in any Salvation Army or inter-agency committee meetings as requested by Program Coordinator or designate
  • Participate in any and all staff training to continuously upgrade skills based on Correctional Services and Salvation Army standards and recommendations
  • Attend all staff meetings
  • Participate as a team member of house staff to support residential and direct support workers
  • Respect all lines of authority TSA and Corrections (and others as deemed necessary)
  • Protect the confidentiality of all residents and staff
  • Ability to cross-task with case management team as required
  • Encourage atmosphere of trust and respect with all residents and staff
  • Exercise appropriate level of control as required

Service Responsibilities

  • Arrive 10 to 15 minutes prior to shift start in order to achieve a smooth transition

Key focus of the role is to effectively manage the caseload

  • Prepare and maintain case management schedule and caseload risk profile
  • Engage in regular case management meetings with Railton Case Managers and Program Coordinator
  • Prepare and present case management reports for approval by Program Coordinator and Parole Officers.
  • Lead and support meetings with Parole Officer and residents to discuss all matters related to case load.
  • Undertake Case Reviews on OMS create reports and present recommendations for selection of incoming residents for Community Assessment Team (CAT) review meetings. Attend meetings as required.
  • Communicate with POs and CSC personnel as required to promote the ongoing progress of caseload
  • Regular engagement with resident case load to support progress against the Resident Action Plan
  • Counsel residents build trust and form effective relationships to support progress
  • Ensure intake of new residents on caseload is completed
  • Keep track of residents eligibility for weekend pass
  • Facilitate prescription referrals with CSC and pharmacy providers
  • Ordering of refills/ new prescriptions of Residents medication
  • Ensure close out tasks for resident case load are completed and all required information is destroyed in accordance with the security requirements
  • Organize and schedule appointments with community partners

Support TSA Bubble staff resident / direct worker activity on shift where needed

  • If covering the resident worker / direct worker Bubble Team shift case worker must complete a full house round to ensure premises are neat clean and make sure proper head count coincides with names and numbers recorded in logbook before accepting keys.
  • Ensure all doors required are locked to ensure security of each resident and the building
  • Read and discuss with outgoing staff any new entries in logbook or any relevant incidents/information reports that require follow-up
  • Monitor Sign-Out Log to verify name and location when resident signs out and upon call-ins
  • Distribute medications and record in medication logbook
  • Assist residents with scheduling medical dental program job interviews etc. appointments
  • Must be awake at all times
  • Support the monthly development of reporting to CSC and The Salvation Army
  • Ensure the portable phone and panic button is always with you
  • Occasional solo shifts where unavoidable due to team absence
  • Participate in on-call rota
  • Assist Program Coordinator as required
  • Perform other duties as deemed necessary by Director or Program Coordinator

Qualifications and Education Requirements

  • Must be able to secure and maintain Canadian Reliability Clearance
  • Microsoft Office literate
  • University Degree in social or behavioral sciences or related field; 1 year working experience in social service field preferably corrections.
  • Effective communication skills both orally and written.
  • Effective conflict resolution and problem-solving skills.

Compensation:

The target hiring range for this position is $19.67 to $24.59 with a maximum of $29.51.

Placement in the salary range will be based on factors such as market conditions internal equity and candidate experience skills and qualifications relevant to the role.
