Position Summary: The Licensed Practical Nurse (LPN)/Registered Nurse (RN) in the Skilled Nursing Facility role is essential in providing skilled nursing care and support to individuals with a variety of medical conditions and goals of care. This position works closely with the interdisciplinary team to ensure the well-being and safety of residents while promoting their overall quality of life. Essential Duties and Responsibilities: - Administer medications and treatments as prescribed by physicians ensuring accurate documentation and adherence to medication management protocols.
- Perform routine nursing assessments including vital signs physical examinations and cognitive assessments to monitor residents health status and detect any changes or concerns.
- Collaborate with the healthcare team to develop and implement individualized care plans based on residents needs and preferences.
- Provide direct nursing care including wound care catheter care and assistance with activities of daily living (ADLs) such as bathing dressing and eating.
- Communicate effectively with residents their families and the interdisciplinary team to ensure a comprehensive and coordinated approach to care.
- Act as a liaison between residents families and healthcare professionals relaying important information addressing concerns and providing emotional support.
- Collaborate with other healthcare professionals such as physicians therapists and social workers to coordinate and integrate care services.
- Document and maintain accurate and up-to-date medical records including nursing assessments care plans and progress notes.
- Implement and enforce safety protocols to maintain a secure and comfortable environment for residents.
- Monitor residents for any signs of distress changes in behavior or medical emergencies and take appropriate action as needed.
- Respond promptly and effectively to medical emergencies following established protocols and procedures.
- Other duties as required.
Knowledge Skills and Abilities - High School Diploma or GED.
- Active LPN/RN license within the state of Kentucky.
- This can include attending workshops seminars and pursuing additional certifications or specialized training in memory care.
- Ability to provide compassionate and personalized care to residents.
- Effective and clear communication
- Knowledge of medication management including proper dosage administration routes and potential side effects.
- Attention to detail and adherence to legal and ethical standards.
- Willingness to collaborate and work as part of a multidisciplinary team.
