• Administer medications and treatments as prescribed by physicians ensuring accurate documentation and adherence to medication management protocols.
• Perform routine nursing assessments including vital signs physical examinations and cognitive assessments to monitor residents health status and detect any changes or concerns.
• Collaborate with the healthcare team to develop and implement individualized care plans based on residents needs and preferences.
• Provide direct nursing care including wound care catheter care and assistance with activities of daily living (ADLs) such as bathing dressing and eating.
• Communicate effectively with residents their families and the interdisciplinary team to ensure a comprehensive and coordinated approach to care.
• Act as a liaison between residents families and healthcare professionals relaying important information addressing concerns and providing emotional support.
• Collaborate with other healthcare professionals such as physicians therapists and social workers to coordinate and integrate care services.
• Document and maintain accurate and up-to-date medical records including nursing assessments care plans and progress notes.
• Implement and enforce safety protocols to maintain a secure and comfortable environment for residents.
• Monitor residents for any signs of distress changes in behavior or medical emergencies and take appropriate action as needed.
• Respond promptly and effectively to medical emergencies following established protocols and procedures.
• Other duties as required.

• High School Diploma or GED.
• Active LPN/RN license within the state of Kentucky.
• This can include attending workshops seminars and pursuing additional certifications or specialized training in memory care.
• Ability to provide compassionate and personalized care to residents.
• Effective and clear communication
• Knowledge of medication management including proper dosage administration routes and potential side effects.
• Attention to detail and adherence to legal and ethical standards.
• Willingness to collaborate and work as part of a multidisciplinary team.
