Partnership Opportunity: Could be a possibility in the future

Target Compensation: $135K/4days a week with full benefits

Bonus Opportunity: Yearly bonuses are currently paid at Employer’s discretion

Retirement Program: 401K w/ matching contribution up to 4%

Other Benefits: Health Insurance Life & Disability Contribution for Continuing Education and professional dues and credentialing/licensure Eyewear stipend

Relocation: $8-10K for relocation assistance

Current patient volume: 3000-3250 patients a year per doctor

Support Staff: Full support staff; ophthalmic techs opticians etc

MDs on Staff: 2 MD’s currently on staff

ODs on Staff: 7 OD’s currently on staff

OD – patient per day expectation: 18-20 patients
