Position Overview:

The Healthy Texas Womens (HTW) Patient Navigator is a front-line public health worker who is a trusted member of and has an unusually close understanding of the communities served by Harris County Public Health. Under the direction of the Clinical Health Services Program Manager the HTW Patient Navigator is responsible for screening and assisting women who qualify with submitting an application for The Healthy Texas Women Program (HTW). Healthy Texas Women programs offers free womens health and family planning services to eligible low income women. Upon submitting applications the Patient Navigator will support the patient with scheduling their first wellness appointment at one of the Clinical Health & Wellness Clinics. This position will support reporting out comes to the State of Texas & Coordinating care needs with the Clinical Health & Wellness team.

Duties & Responsibilities:

  • Responsible for a variety of outreach education and community coordination activities including canvasing apartment complexes community events and business outreach.
  • Responsible for eligibility screening and supporting applications for The Healthy Texas Womens program.
  • Collecting all needed documentation from patients and uploading them within a timely manner.
  • Scheduling patients first wellness exam in the Electronic Medical Records system.
  • Call and coordinate in person eligibility visits to complete applications which can include home visits.
  • Maintains a calendar of appointments and outreach activities for reporting purposes.
  • Working closely with the Clinical Health Services team on grant reporting grant metrics and follow up of applicants and ensuring positive outcomes.
  • Other duties as assigned including special tasks involved in responding to an emergency event.

Requirements

Education:

  • High school diploma or some college education.

Experience:

  • At least two (2) years of experience performing clerical office duties in a health care setting (hospital physician or dental office).

Knowledge Skills & Abilities:
  • Proficient in MS Office Suite (Word Excel PowerPoint Outlook).
  • Strong understanding of cultural competency.
  • Understands ethics and public good; is concerned with public trust.
  • Demonstrates respect for the opinions and beliefs of others.
  • Excellent communication skills (verbal & written).
  • Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment.
  • Ability to maintain confidentiality when dealing with sensitive information.

General Information

Position Type and Typical Hours of Work

  • Full-Time Grant-funded
  • Monday Friday 8 am 5 pm with an occasional Saturday.
  • Other days and hours as necessary to meet the needs of team.

Work Environment

  • This job operates in both a community setting and clinical office environment. This role routinely uses standard office equipment such as computers phones photocopiers filing cabinets and fax machines..

Physical Demands

  • This role requires the ability to lift carry push pull up to 20 pounds and bending & standing for periods at a time.
