Position: Occupational Therapist (OT)
Shift: Day Shift (8:00 AM 4:30 PM)
Meal Break: 30 minutes
Contract Duration: 13 weeks
Weekly Guarantee: 35 hours
Schedule: Sunday Saturday (7-day coverage rotation)
Role Overview
The Occupational Therapist will provide rehabilitative therapy services in a clinical or long-term care environment. The role focuses on improving patients functional abilities safety and independence through individualized treatment plans.
Key Responsibilities
- Evaluate patients to determine functional status therapy needs and treatment goals.
- Develop and implement individualized care plans and therapeutic interventions.
- Provide rehabilitative therapy to improve mobility strength coordination and daily living skills.
- Document evaluations progress notes and discharge summaries in accordance with regulatory and facility standards.
- Collaborate with an interdisciplinary team including PTs SLPs nurses and caregivers.
- Educate patients and families on home exercise programs and ongoing functional strategies.
- Participate in care planning meetings and contribute to patient progress discussions.
- Ensure compliance with all facility policies state regulations and professional standards.
Qualifications
- Active state license as an Occupational Therapist.
- Experience in skilled nursing outpatient rehab or similar settings preferred.
- Strong assessment and clinical decision-making skills.
- Ability to work a full 7-day rotating schedule as needed.
- Excellent communication documentation and teamwork abilities.
