Job Summary

Position: Occupational Therapist (OT)
Shift: Day Shift (8:00 AM 4:30 PM)
Meal Break: 30 minutes
Contract Duration: 13 weeks
Weekly Guarantee: 35 hours
Schedule: Sunday Saturday (7-day coverage rotation)

Role Overview

The Occupational Therapist will provide rehabilitative therapy services in a clinical or long-term care environment. The role focuses on improving patients functional abilities safety and independence through individualized treatment plans.

Key Responsibilities

Evaluate patients to determine functional status therapy needs and treatment goals.
Develop and implement individualized care plans and therapeutic interventions.
Provide rehabilitative therapy to improve mobility strength coordination and daily living skills.
Document evaluations progress notes and discharge summaries in accordance with regulatory and facility standards.
Collaborate with an interdisciplinary team including PTs SLPs nurses and caregivers.
Educate patients and families on home exercise programs and ongoing functional strategies.
Participate in care planning meetings and contribute to patient progress discussions.
Ensure compliance with all facility policies state regulations and professional standards.

Qualifications

Active state license as an Occupational Therapist.
Experience in skilled nursing outpatient rehab or similar settings preferred.
Strong assessment and clinical decision-making skills.
Ability to work a full 7-day rotating schedule as needed.
Excellent communication documentation and teamwork abilities.
