Job Description (Client Name Hidden)

We are seeking a reliable and compassionate Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) for an evening-shift assignment in Montour Falls NY. The LPN/LVN will provide high-quality nursing support under the supervision of registered nurses and physicians while assisting with both clinical tasks and basic patient-care duties as needed.

Key Responsibilities:

• Provide direct patient care in accordance with nursing best practices and facility protocols.

• Monitor and document patient conditions vital signs and responses to treatment.

• Administer medications and treatments as prescribed by licensed providers.

• Assist with basic nursing assistant duties including bathing grooming mobility support and toileting.

• Report changes in patient condition promptly to supervising staff.

• Maintain accurate and timely documentation within the electronic health record (EHR).

• Follow infection control and safety procedures at all times.

• Participate in weekend and holiday rotations as scheduled.

• Collaborate with the healthcare team to support patient care plans and unit workflow.

Requirements:

• Active LPN/LVN license (state-specific licensure required or compact eligibility if applicable).

• Minimum 612 months of recent clinical experience (may vary by facility).

• BLS certification (required).

• Strong communication observation and patient-care skills.

• Ability to assist with CNA-level responsibilities when needed.

• Willingness to work evening shifts and rotating weekends/holidays.
