Job Summary:

Performs scheduled in-home clinical care and post-discharge follow-up as part of the Mobile Integrated Healthcare (MIH) program. Performs home safety inspection patient assessment medication reconciliation point-of-care testing telehealth facilitation and equipment training to support safe transition of care and reduce readmissions.

Position Responsibilities:

Conducts in-home patient assessments following discharge to evaluate condition medication adherence and safety risks. Reports findings to supervising nurse or provider as appropriate.

Completes accurate clinical documentation in the EMR/ePCR. Responds to quality assurance and medical oversight requests.

Facilitates telehealth visits with providers and communicates findings to the interdisciplinary care team.

Operate and monitor designated electronic equipment (e.g. vitals monitors tablets telehealth tools) per training and within EMT scope.

Communicates professionally with patients families and multidisciplinary team members to coordinate care and escalate concerns.

Position Qualifications Required / Experience Required:

Minimum 1 year of active field experience as a New Jersey-certified EMT required.

Experience in home healthcare or mobile integrated healthcare is highly preferred.

Required Education:

High school diploma or GED.

Graduate of a recognized EMT Training Program.

Training/Certifications/Licensure:

Current New Jersey EMT certification issued by the NJ Department of Health

Current BLS CPR certification (American Heart Association or equivalent).

Valid drivers license recognized by the State of New Jersey; driving record must meet insurance company standards.

Must complete annual clinical competencies Fit Test PPD screening AMOC training and any other required training by designated deadlines.
