Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute sub-acute aged care and community services.

NHW have an exciting opportunity for an experienced Registered Nurse or Allied Health Professional with a passion for collaborative patient care to join our Post Acute Care team as a clinical Care Coordinator.

The Care Coordinator - PAC provides comprehensive clinical assessment needs identification and service coordination to our clients following an hospital stay. The role provides virtual care to clients with face-to-face care delivered by brokered providers. The successful applicant will join a well-established and supportive team and will work closely with referrers and community service providers to ensure patients receive appropriate supports after hospitalisation.

Wangaratta has a population of approximately 30000 and our catchment serves a community of around 100 000 throughout the northeast.
An easy 2 ½ hours drive on the Hume freeway from Melbourne and less than an hour to Albury airport with direct flights to most of the large Australian cities.
