DescriptionProvides medication management services in coordination with other health care professionals to deliver safe appropriate and cost-effective medication therapy to patients. Participates in collaborative practice and/or protocol-driven practice as appropriate. Specifically the Clinic Outpatient Pharmacist provides high-quality pharmaceutical care through the accurate interpretation and dispensing of medication orders in the outpatient pharmacy close collaboration with providers and allied health staff and direct patient engagement to ensure safe and effective medication therapy. QualificationsRequired Qualifications
- Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program OR Bachelor of Science (B.S.) AND a minimum of 2 years relevant experience.
Preferred Qualifications
- Other advanced degrees (e.g. Masters Ph.D.) and applicable Board of Pharmacy Specialties (BPS) or other certification applicable to the position as determined by the supervisor.
- Completion of pharmacy residency and specialization (e.g. PGY-2 and respective BPS certification) if applicable.
- Evidence of ongoing scholarly pursuits including research/quality improvement publications or scientific presentations.
- Relevant applicable pharmacy experience working in a progressive and collaborative practice environment.
- Experience and/or interest in teaching and/or precepting.

- Commitment to excellence accuracy attention to detail and teamwork required.
- Ability to communicate effectively work collaboratively with others organizes time well practice with an agile mindset solve problems and work independently with minimal supervision required.
License and Certification
