Key Responsibilities:
Evaluate plan and implement individualized physical therapy treatment programs
Provide hands-on therapy exercises and modalities to improve patient mobility strength and function
Monitor patient progress and adjust treatment plans as necessary
Document patient care progress notes and treatment outcomes according to facility policy
Collaborate with interdisciplinary team members to ensure optimal patient care
Educate patients and families on therapeutic exercises and injury prevention
Qualifications:
Active Physical Therapist license in Connecticut
Minimum 12 years of experience in post-acute care skilled nursing or rehabilitation settings required
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Excellent clinical assessment and problem-solving abilities
