Job Summary:
The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration skills are essential for working with patients providers and team members.
Location:Santa Rosa California United States
Responsibilities:
- Provide skilled nursing care to patients.
- Conduct patient assessments and monitor their condition.
- Administer medications and treatments as prescribed.
- Develop and implement patient care plans.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Communicate effectively with patients families and healthcare providers.
- Document patient information accurately and timely.
- Float between like units as needed.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Minimum 1 year of relevant experience.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and automobile for field work.
- AHA BLS Provider CPR
Preferred Skills & Certifications:
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room.
Special Considerations:
- Weekend work is required.
- On-call may be required.
- Royal Blue or Navy blue scrubs required.
Scheduling:
- Variable start times: 5am 6am 10:30am or 3:30pm.
- Shift assignments and block scheduling are per manager discretion.
