Description
We are seeking a compassionate motivated Full Time Residential Care Coordinator to join our residential treatment team and support individuals beginning their recovery journey from substance use. This role plays a vital part in engaging clients coordinating treatment and aftercare collaborating with referral sources and community partners and supporting both clients and families during the early stages of recovery.
Key Responsibilities
- Build and maintain strong clinical relationships with clients and families entering residential treatment.
- Assess client goals psychosocial needs financial considerations and discharge planning requirements.
- Create facilitate and monitor comprehensive aftercare plans through interdisciplinary collaboration.
- Act as liaison between staff healthcare providers referral sources caregivers and payors.
- Identify and resolve barriers to a timely and successful discharge.
- Ensure clients and families understand all planned aftercare services and related costs.
- Maintain accurate and timely documentation in the EMR.
- Participate in cross-training and assist as a Substance Use Disorder Counselor.

Work Environment & Physical Requirements
- Work is performed in a quiet office-style environment with travel required within the service area.
- Ability to sit stand walk communicate and use office equipment (keyboard phone etc.).
- Must occasionally lift/move up to 10 pounds.

Requirements
- An associate degree in a human services-related field with 1-2 years of relevant experience in the field of substance use disorder is required. A bachelors degree in a Human Services-related field(s) is preferred.
- Licensure as a Certified Alcohol and Drug Counselor (CADC) or as a Licensed Alcohol and Drug Counselor (LADC) is preferred at the time of hire however the agency is willing to provide clinical consultation and support to obtain the required licensing.
- Holders of this position must complete a physical at least once every three years.
