36 hour per week PM RNs 8pm-830am every other weekend
POSITION QUALIFICATIONS/REQUIREMENTS:
Education and/or Experience: Associates degree (A.A.) or equivalent from two-year college or technical school. Minimum of 1 year of current active nursing experience; with experience in caring for terminally ill patients or those with life threatening illnesses preferred; prefer one (1) year experience in an acute or post acute care setting. An equivalent of education and experience may be considered.
Licensure:
o Valid Florida license as a Registered Nurse
o Valid Florida State Drivers License
Continuing Education: As required for licensure.
Certifications: Valid CPR certification from the American Red Cross or American Heart Association
The ability to type 25 wpm is highly preferred. Must receive a passing grade on a pre-employment computer literacy evaluation.
Must be able to remain flexible and work at alternate facility where there is a business need for reassignment for one or more shifts at an alternative facility. When this occurs the colleague is responsible for their own transportation to the alternate facility and at their expense. If mode of transportation is self-driving to that facility the colleague must maintain an active valid drivers license and proof of valid automobile insurance both of which will be placed in the colleagues HR file.
Physical Requirements:
Ability to lift pull push 50 lbs. Also needs to have physical stamina to meet the demands of long shifts and physically demanding tasks.
Ability to ambulate throughout facility and stand for long periods of time.
Ability to bend squat kneel and reach.
Ability to make coordinated hand and finger movements and possess strong coordination and balance.
Visual acuity and ability to hear in order to auscultate patients hear alarms view monitors etc.

JOB DUTIES/RESPONSIBILITIES:
1. Supports and carries out the organizations mission vision and values on a consistent basis.
2. Demonstrates the ability to obtain patient history perform an age-specific physical psychosocial and spiritual assessment and initiates appropriate referral intervention and collaboration between the patient family Interdisciplinary team (IDT) physician and partnering facility (as applicable) following policy and practice guidelines.
3. The RN will identify patient/family goals develop a Plan of Care with patient family IDT attending physician and/or partnering facility and continually evaluate progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change (RN performs as case manager).
4. The RN will provide collaboration and care coordination between the patient family Interdisciplinary team (IDT) physician and partnering facility (as applicable) following policy and practice guidelines.
5. The RN will use excellent critical thinking judgment and ethical principles for problem solving positive and effective collaboration and follow-through in all patient and caregiver clinical situations.
6. The RN will demonstrate the ability to administer monitor and document medications via all routes using age-specific care practices and provide appropriate patient/family teaching
7. The RN will identify age-specific patient and caregiver learning needs motivations barriers abilities and preferences and utilize teaching strategies/instructions and follow-through to match.
8. The RN will promote a culture of safety by implementing policy guidelines and clinical documentation applying best practice safety assessment and management principles and providing appropriate patient/family teaching and follow-through.
9. The RN will adhere to principles of infection control including Standard Precautions on all patients use appropriate isolation & PPE perform proper hand hygiene and provide appropriate patient/family teaching.
10. The RN will prepare for and participate in the collaborative dialogue surrounding the Plan of Care during the IDT meeting valuing the expertise of all disciplines.
11. The RN will document all care processes within the EMR in a timely manner according to policy and practice guidelines.
12. The RN will follow palliative care and end of life principles for all interactions and provide compassionate care adhering to policy and guidelines.
13. The RN directs and oversees the care provided by the LPN and Home Health Aide following agency policy and Care Center protocols. The RN collaborates with Care Center nursing leadership re: changes in patient condition pt/family concerns nursing staff needs
14. The RN demonstrates knowledge of Empath Health policies guidelines and clinical procedures and pertinent State and Federal Rules and Regulations and accreditation standards.
15. The RN participates in the team responsibility of providing 24/hour a day 7 days a week patient care.
16. The RN attends inservice and educational programs including all mandatory meetings trainings and skills review.
17. The RN assumes personal responsibility for maintaining up to date credentials and Continuing Education requirements including CPR licensure and TB.
18. The RN supports the overall provision of care in the Care Center by adhering to approved staffing ratios and maintaining Care Center competencies. The RN supports the operations of the Care Center by participating in and supervising others in disaster preparedness fire safety regulatory inspections and surveys. The RN performs other duties as requested by supervisor.
