Position Overview:
Navitas Healthcare LLC is seeking a Physical Therapist for a job in Brooklyn NY.

Responsibilities:

  • Conduct comprehensive patient evaluations and develop individualized treatment plans.

  • Provide therapeutic exercises manual therapy and functional mobility training.

  • Educate patients and caregivers on home exercise programs and rehabilitation goals.

  • Document assessments progress notes and therapy outcomes accurately and timely.

  • Collaborate with physicians and interdisciplinary rehab teams to ensure coordinated patient care.

  • Adhere to facility policies regulatory requirements and professional standards.

Requirements:

  • Active Physical Therapist license in New York.

  • Graduate of an accredited Physical Therapy program.

  • Strong clinical assessment and therapeutic skills.

  • Ability to manage an independent caseload effectively.

  • Excellent communication and patient-education abilities.
