Position Overview:
Navitas Healthcare LLC is seeking a Physical Therapist for a job in Brooklyn NY.
Responsibilities:
Conduct comprehensive patient evaluations and develop individualized treatment plans.
Provide therapeutic exercises manual therapy and functional mobility training.
Educate patients and caregivers on home exercise programs and rehabilitation goals.
Document assessments progress notes and therapy outcomes accurately and timely.
Collaborate with physicians and interdisciplinary rehab teams to ensure coordinated patient care.
Adhere to facility policies regulatory requirements and professional standards.
Requirements:
Active Physical Therapist license in New York.
Graduate of an accredited Physical Therapy program.
Strong clinical assessment and therapeutic skills.
Ability to manage an independent caseload effectively.
Excellent communication and patient-education abilities.
