About the Role
As a Community Nurse youll play a vital role in supporting clients with a wide range of needs including complex care rehabilitation and end-of-life support. Working closely with clients families and multidisciplinary teams youll help create safe personalised and effective care plans that make every day better for those we support.
What Youll Do
Deliver high-quality person-centred nursing care
Complete assessments care plan and evaluate outcomes
Support clients with complex health needs in their homes
Administer medication safely and accurately
Offer guidance support and training to staff
Promote health wellbeing dignity and independence
Maintain accurate clinical documentation
Contribute to continuous improvement and quality standards
What Were Looking For
Registered Nurse (NMC PIN required)
Experience in community acute or complex care settings (all experience levels considered)
Strong communication and clinical decision-making skills
Compassion reliability and a commitment to delivering outstanding care
Ability to work independently and as part of a supportive team
A flexible proactive approach
