Schedule: 40 hours/week MondayFriday 9:00 AM5:00 PM
Pay Rate: $55$58 per hour
Job Summary:
Navitas Healthcare LLC is seeking Licensed Master Social Workerfor an exciting job in New York NY.
Responsibilities:
Conduct psychosocial assessments and develop individualized service or treatment plans.
Provide counseling crisis intervention and emotional support to clients.
Coordinate referrals and collaborate with healthcare providers community agencies and support services.
Monitor client progress and modify care plans as needed.
Maintain accurate timely documentation in accordance with agency and state guidelines.
Participate in interdisciplinary team meetings and contribute to care planning discussions.
Advocate for clients and assist them in accessing necessary resources.
Ensure compliance with all ethical legal and professional standards of social work practice.
Required Skills & Experience:
Minimum 1 year of experience as a Licensed Master Social Worker.
Education:
Masters Degree in Social Work (MSW) required.
Certifications & Licensure:
Active LMSW license issued by NYSED (Primary Source Verification required).
