Public HealthSeattle & King Countys (PHSKC) Health Care for the Homeless Network (HCHN) Street Medicine Team is hiring a part-time (0.8 FTE) Street Medicine Nurse(Public Health Nurse)to join our team. We are seeking qualified candidates who have experience in providing nursing care in ambulatory care and outreach settings as well as comfort with practicing to the top of the Registered Nurse (RN) scope of practice as a member of a multidisciplinary team. Nurses with lived experience and/or who hold marginalized identities (e.g. BIPOC LGBTQ2S) are encouraged to apply.

Position Summary

In this role the nurse will join a multidisciplinary Street Medicine Team that addresses the medical behavioral health and harm reduction needs of people living homeless. This RN will have a weekly schedule that includes street outreach as well as shifts at our low-barrier pocket clinic located inside of the ETS-REACH Markham drop-in space in Belltown. This role may occasionally provide support to other current or future Mobile Medical Teams providing float coverage as well as to increase capacity for conducting nurse visits.


Background

CHS is the largest of 7 divisions in Public Health Seattle & King County with over 500 employees and deliver a variety of health services through the programs partnerships and Public Health Centers they operate. We deliver direct care through Public Health Centers and partners with community agencies to provide medical dental WIC family planning and care coordination services. Its mission is to ensure equitable access to health care especially for low-income and underserved populations. We serve a diverse low-income population including people who are experiencing homelessness recent immigrants and refugees people with complex health conditions and limited English proficiency. We are Mission-Critical employees who are dedicated to showing up and providing the best care under any circumstances.

Within CHS the Healthcare for the Homeless Network focuses on people experiencing homelessness in King County offering medical dental behavioral health case management and respite care. The Street Medicine Team seeks out and delivers medical care behavioral health care and social services directly to people living unsheltered across King County. Care is provided in encampments on sidewalks at drop-in spaces and in collaboration with homeless service organizations. The Street Medicine team aims to engage people in health education professional and peer support and treatment for their medical and behavioral health conditions via a trauma-informed antiracist and harm reduction approach.

This part-time position will be a 0.8 full-time equivalent (FTE). A typical work week is Monday through Thursday 8:00 am to 5:00 pm and may be occasionally required to work outside normal hours or on Saturday. This position is designated as hourly/non-exempt under the regulations of the Fair Labor Standards Act (FLSA) which means it is overtime-eligible. Staff must receive prior approval from their supervisor before working any necessary overtime hours to meet their position responsibilities.

South Park Office 7272 W Marginal Way S Seattle WA 98108. This position may be required to cover at our other Public Health Centers located throughout King County.

Work Conditions
Work is conducted in a variety of locations including indoors in office in a vehicle and outdoors. Travel throughout King County to various locations where unsheltered people are living is required. Outreach work is conducted in uncontrolled outdoor environments and will require the employee to traverse over uneven surfaces and obstacles ascend and descend slopes and move through wooded areas without maintained trails while transporting gear weighing up to 35lbs. The employee will frequently work in and around extreme heat and cold precipitation ambient smoke loud noises individuals who are distressed intoxicated or displaying unpleasant behavior and potentially biohazardous material. Must constantly perceive surroundings for and communicate with the team about potential hazards under these conditions. Appropriate training and PPE will be provided. Must be able to communicate clearly and quickly with team members and clients in English and be responsive to phone calls and messages throughout the day. Must be able to remain calm in adverse conditions and respond appropriately to emergency situations following established safety protocols. The employee may be called upon to administer naloxone and/or provide Basic Life Saving (BLS) care to individuals in emergency situations alone or as a member of a team. The employee will be required to share responsibility for driving the outreach vehicle(s) up to 60 miles per day in all weather conditions. The employee will also be required to move boxes supplies and packs weighing up to 35lbs between ground level and 5ft to load and unload outreach supplies.

Job Duties

  • Provide outreach and engage with people living homeless in community settings such as encampments sidewalks parking lots day centers and homeless service sites.
  • Triage and assess a wide variety of medical behavioral health and psychosocial needs.
  • Provide comprehensive patient education and support for sexual and reproductive health care substance use disorder harm reduction practices and chronic disease management.
  • Safely administer and dispense medications in accordance with policy and professional standards.
  • Document patient care in electronic medical record (Epic) Facilitate linkages to primary behavioral health and specialty care.
  • Track new referrals and patient follow-up needs; participate in daily huddle to plan outreach encounters based on acuity.
  • Provide expertise regarding public health nursing scope practice and policy at individual programmatic and community levels.
  • Participate in program evaluation and planning; continually tailor services and operations to provide accessible high quality culturally relevant care for unsheltered populations Participate in quality assurance activities to meet laws regulations policies and procedures.
  • Provide training orientation and education to staff clients students and the community.
  • Adhere to safety policies and procedures. Participate in professional development opportunities.
  • Attend staff meetings case conferences and emergency drills.
  • Provide birth control and pregnancy options counseling (including referrals for pregnancy termination services).
  • Train clients on the use of naloxone and dispense naloxone.
  • Assess and counsel clients regarding opiate use disorder and medication assisted treatment.
  • Share responsibility with other Street Medicine Team staff to drive outreach vans. Training is provided for driving county vehicles.
  • Other duties as assigned.

Experience Qualifications Knowledge Skills

Our ideal candidate willdemonstratethe following(Required Qualifications):

  • Washington State license as a Registered Nurse (in good standing) and a bachelors degree (or higher) in nursing science from a School of Nursing accredited by the National League of Nursing.
  • Documentation of current BLS/CPR certification with AED training required at time of hire. (CPR/AED Heartsaver courses do not meet this requirement.)
  • Valid Washington State Drivers License.
The most competitive applicants will have the following experiences and qualifications(Desired):
  • Bilingual in Spanish/English.
  • Experience providing health education and nursing care to individuals who are living homeless with substance use and mental health conditions.
  • Ability to apply knowledge of harm reduction trauma-informed care and equity & social justice principles to nursing practice.
  • Outreach and/or lived experience in encampment settings.
  • Ability to establish rapport with diverse marginalized communities and individuals.
  • Ability to work in a fast-paced environment independently manage time and handle multiple tasks.
  • Personal flexibility a sense of humor and the ability to accept feedback while working with a highly collaborative team.
  • Experience in verbal de-escalation and physical interventions in emergent situations to maintain a safe and secure environment.
