About the Role: Certified Home Health Aide (CHHA)
As a CHHA youll be providing essential personal care and support services in the comfort of your patients homes. Youll follow an individualized care plan under the direction of a registered nurse or therapist helping patients with daily tasks while preserving their dignity and independence.
Key Responsibilities:
Personal Care Assistance: Help patients with bathing grooming dressing and toileting
Mobility Support: Assist with transfers walking exercises and fall prevention
Monitor Vital Signs: Check and document temperature pulse and respiration during each visit
Meal Preparation: Assist with meals and feeding based on the patients dietary plan
Medication Reminders: Prompt patients to take prescribed medications (no administration required)
Light Housekeeping: Change bed linens keep the environment tidy and safe
Observe & Report: Monitor patient condition and communicate changes to clinical staff
Follow Care Plan: Deliver care based on each patients personalized home health care plan
Requirements:
Current CHHA Certification through the California Department of Public Health
Reliable Transportation Youll be traveling to patients homes from Calabasas Studio City Sunland Chatsworth San Fernando & Surrounding Areas
CPR Certification Current and valid
Strong Communication Skills Fluent in spoken and written English
Empathy & Professionalism Compassionate respectful and dedicated to patient-centered care
Home Health Experience Preferred but not required
