What Youll Do in This Role:

Join our team as a Home Care Specialist and play a key role in supporting participants care from behind the scenes. Youll coordinate and schedule home care services manage aide schedules and keep communication flowing between staff participants and coordinators. This position also provides administrative support processes approvals for home care hours DME and supplies and assists with services during inclement weather. Its a great opportunity to make a meaningful impact while ensuring smooth and efficient home care operations.

Primary Responsibilities:

Home Care Coordination Support

  • Assist home care coordinators in scheduling and coordinating home care services in alignment with participant care plans
  • Coordinate home care support with agency staff as directed

Home Health Aide Scheduling

  • Ensure maximum productivity of home health aide schedules based on participant needs
  • Communicate schedule changes promptly to staff participants and coordinators

Administrative Support & Documentation

  • Provide administrative support for maintaining accurate home care records in the Electronic Health Record (EHR)
  • Complete prior authorizations for home care durable medical equipment (DME) and supplies

Utilization Review & Invoice Processing

  • Conduct utilization review and reconcile for timely invoice processing adhering to organizational standards

Scheduling Management

  • Assign and manage home care aide schedules with home care coordinators to meet participant needs efficiently

Attendance & PTO Tracking

  • Track attendance and paid time off (PTO) for staff as assigned

Staff Performance & Development

  • Participate in performance reviews and collaborate with supervisors on home care staff development

Relationship Development

  • Develop and maintain effective relationships with contracted staffing agencies to ensure high-quality service delivery

Standard Operating Procedures (SOPs)

  • Assist in creating and updating SOPs for assigned work as directed by supervisors

What Youll Need:

Education & Certification

  • High school diploma or equivalent required
  • Associate or bachelors degree in healthcare social work or a related field preferred.
  • CPR certification or willingness to obtain

Experience

  • Prior experience in scheduling coordinating or managing home care services with a solid understanding of home care coordination processes.
  • 2-3 years of experience in a healthcare related field particularly in a home care or participant-facing setting.
  • At least 1 year of experience working with the elderly or in geriatric care settings.
