Transition Coordinators play a vital role in helping youth prepare for reentry into schools vocational programs and communities. By guiding students through education planning vocational training and reintegration services this position directly supports OJJs mission to promote growth accountability and positive futures.
The WAE Transition Coordinator will serve as a bridge between students families schools and community partners ensuring that every youth has the tools resources and support needed to thrive after release.
Job Duties:
The Transition Coordinator supports students in successfully reintegrating into schools communities and vocational programs.
Key responsibilities include:
Student Transition Planning (40%)
Coordinate education and reintegration plans with social services staff and families.
Prepare release packets for students parents and local school districts.
Develop transition lessons and collaborate on vocational education issues.
Collaboration & Meetings (20%)
Participate in staffings and IEP meetings to address transition needs.
Network with other transition coordinators and local education agencies.
Assist with scheduling vocational adult education and GED preparation services.
Family & Community Engagement (20%)
Contact parents for enrollment information and maintain documentation of all contacts.
Build community resource guides and regional contacts to support student re-entry.
Help teachers and students develop portfolios and deliver classroom presentations on transition topics.
Policy & Professional Standards (15%)
Adhere to OJJ policies and procedures.
Serve on committees or task forces as designated.
Maintain professional relationships with students staff and interdisciplinary teams.
