Job Summary
Transition Home Healthcare is an elite home health agency serving Houston TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients families team members and the community. Our team is growing and looking for a dedicated:
Physical Therapist(PT)
Shifts Available:Full-Time &Part Time
Service Areas with Openings: Southeast (Pearland)
Responsibilities:
- Facilitate the rehabilitation of patients with emotional physical or mental disabilities.
- Implement physical therapy care plans.
- Coordinate care with the home healthcare team.
- Educate patients and family members about health conditions.
- Prepare timely complete and accurate clinical notes.
- Communicate effectively with patients caregivers healthcare providers office support and other care team members.
Qualifications:
- Physical Therapist in the state of Texas.
- Home health experience highly preferred.
- Knowledge of home health regulations and general healthcare compliance.
- Experience with HCHB EMR. (preferred but not required)
- Able to handle physical workload.
- Good driving record.
- Excellent communication and people skills to effectively interact with patients families and healthcare team members.
