Schedule: Flexible schedule (days/availability coordinated with facility); no holiday coverage required
Position overview
The Physical Therapist SNF provides evaluation and treatment services for residents in a skilled nursing facility focusing on improving mobility function and safety. This role is well-suited to therapists who value patient-centered geriatric rehabilitation interdisciplinary collaboration and managing an independent caseload in a post-acute setting.
Key responsibilities

Evaluate residents and develop individualized physical therapy plans of care including goals and treatment interventions.

Provide therapeutic exercises gait training balance training and functional mobility interventions to improve safety and independence.

Document patient assessments daily treatments progress and outcomes accurately and in a timely manner in the facilitys record system.

Collaborate with nursing occupational therapy speech therapy and other interdisciplinary team members to support resident goals and discharge planning.

Educate patients and caregivers on home exercise programs safe mobility strategies and fall-prevention techniques.

Ensure compliance with facility policies payer requirements and professional practice standards including ethical and evidence-based care.
Required qualifications

Active current Michigan Physical Therapist license.

Experience in a Skilled Nursing Facility preferred; motivated newer PTs with strong clinical skills may be considered.

Strong patient assessment treatment planning and clinical reasoning skills.

Effective communication and interpersonal abilities with residents families and interdisciplinary staff.

Ability to manage a caseload independently prioritize treatments and meet productivity and documentation expectations.

Professionalism reliability and a demonstrated commitment to high-quality patient-centered care.
