Job Summary
RN Multi-Site Director of Case Management
San Diego County California
The RN Director of Case Management leads clinical excellence initiatives across the health system overseeing Case Management Social Services Utilization Review and Clinical Documentation Improvement. This role drives efficient resource use optimal patient flow and financial viability in a dynamic acute care environment.
Qualifications:
- Registered Nurse (RN) license required
- Bachelors Degree required
- Masters Degree (or in progress) required
- Director-level experience in Case Management within an acute hospital setting required
- CCM (Certified Case Manager) or ACM (Accredited Case Manager) certification preferred
Responsibilities:
- Manage appropriate inpatient placement and patient flow through the Continuum of Care
- Coordinate Social Services support for mandatory reporting and patient assistance needs
- Conduct ongoing review and improvement of clinical documentation for quality patient care
- Develop and mentor a high-performing team across all areas of responsibility
- Ensure financial viability by managing revenue expenses supply costs and productivity in cost centers
- Coordinate directly with Managed Care Organizations for efficient care delivery and appropriate reimbursement
- Develop and implement cost-saving measures related to Length of Stay (LOS) and Utilization Review processes
