We are a growing home health company seeking an energetic and results-driven Part-Time Client Case Coordinator to spearhead our client acquisition and service coordination efforts in the Tuscarawas County area. Compensation Structure
This position features a hybrid pay model designed to reward both effort and success:
Base Hourly Rate: $18.00 - $20.00 per hour
Commission: Highly attractive commission paid per new successfully onboarded client in the Tuscarawas County territory. (Details of the commission structure will be discussed during the interview process.)
Transition: A clear path to Full-Time Exempt status (and a competitive salary) upon achieving and maintaining agreed-upon client growth and retention metrics.
Position Purpose
The Case Coordinator is responsible for the coordination and implementation of client services and will have a strong focus on new client acquisition in the designated territory. Principal Accountabilities and Essential Functions (Growth & Coordination). This role requires a blend of sales/growth drive and clinical/administrative oversight. Client Growth & Acquisition (Commission-Driven Focus):
Aggressively follow up on all lead calls and conduct engaging telephone and face-to-face interviews with prospective clients in Tuscarawas County.
Meet with new clients and their families to accurately complete all admission paperwork review services and cost of care and secure the client service agreement.
Maintain positive relationships with potential referral sources and clients in the target area to generate goodwill for the Agency. Case Coordination & Quality:
Develop the Plan of Care in accordance with client needs and wishes and update as warranted.
Schedule conduct and document initial and ongoing client and caregiver meetings including home visits collecting client feedback.
Conduct supervisory visits in the home and ensure services are provided in accordance with the Plan of Care.
Independently respond to effectively resolve and document all client complaints and incidents.
Assess and participate in caregiver performance reviews recognize good job performance and administer disciplinary action when necessary.
Perform on-call evening and weekend duties on a rotational basis as directed and manage calls per company policies.
Required Job Knowledge and Skills
Associate Degree in a health care related field and two years of supervisory experience in homecare is preferred.
Demonstrated ability to listen and communicate clearly fluently and diplomatically both orally and in writing.
High proficiency with Microsoft Office applications (Word Excel Outlook) and relevant scheduling/healthcare software.
Must be a self-starter who can work independently and proactively with minimal direction to meet deadlines.
Must have a valid drivers license and reliable transportation for frequent travel within Tuscarawas County.
Maintain absolute confidentiality of all client and employee information.
