As a Hospice Licensed Practical Nurse heres a big-picture view of what youll do:
- Provide home-based care to patients with illnesses injuries and chronic terminal diseases under the supervision of an RN
- Work with a team of physicians RNs CNAs and aides to deliver quality patient care
- Take patient vitals do assessments document progress and report changes to RN supervisor
- Assist RN in carrying out patients plan of care and teaching patient self-care techniques
- Educate family on the patients plan of care medication and home safety
- Collaborate with interdisciplinary team to coordinate patient care and plan for discharge
A few must-haves for Hospice Licensed Practical Nurses:
- Graduate of an accredited practical nursing program and active LPN license in Kansas
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing strong communication skills and compassion
