Qualifications:

Education: MD/DO degree from an accredited medical school.
Licensure/Certifications:
- Active New York State Medical license.
- Board Certified or Board Eligible in Internal Medicine Family Medicine or Primary Care.
Experience: Open to all levels (negotiable based on expertise).
Language: Bilingual in Spanish preferred.
Technical Skills: Proficiency in EHR systems chronic disease management and preventive care.
Soft Skills: Cultural competency strong communication and a passion for collaborative patient-first care.

Key Responsibilities:

Provide comprehensive primary care including diagnosis treatment and management of acute/chronic conditions.
Collaborate with specialists to deliver integrative multidisciplinary care.
Educate patients on preventive health lifestyle modifications and disease management.
Maintain accurate and timely medical documentation.
Participate in quality improvement initiatives to enhance patient outcomes.
