Location: New York NY 10011
Schedule: 24 hrs/week (WednesdayFriday 9:00 AM5:00 PM)

Pay: $42/hour

Overview:
Navitas Healthcare LLC is seeking a compassionate Licensed Master Social Worker (LMSW) to support patients and families in addressing social emotional and financial challenges related to healthcare needs. This role involves assessment counseling discharge planning and coordination with community resources.

Responsibilities:

Conduct psychosocial assessments and develop care plans.
Provide counseling and connect patients to community services.
Coordinate discharge and referrals for continued care.
Collaborate with healthcare teams for integrated care.
Maintain accurate documentation and participate in quality initiatives.

Qualifications:

Masters Degree in Social Work (MSW) required.
Active LMSW license (LCSW preferred).
Healthcare or medical social work experience a plus.
Strong communication problem-solving and organizational skills.
Proficient in MS Office and EHR systems.
