In accordance with the Vision Purpose and Values and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such the requirement to continuously improve quality and safety is inherent in all aspects of this position.

The Registered Nurse (RN) Healthcare System Navigator reports directly to the Manager or delegate and practices in accordance with relevant provincial legislation nursing standards the code of ethics as outlined by the BC College of Nurses and Midwives employer policies and individual competencies. Provides nursing care in accordance with established organizational polices procedures guidelines and other controls on practice.

The RN works as part of an interdisciplinary clinic team in collaboration with the patient and the patients health care team including primary providers and specialist to provide a culturally safe point of care access for patients who are booked for elective Hip & Knee total joint replacement or other surgery/medical collaboration with members of the Hip & Knee Centre OR Booking and Pre-Admission Clinic teams provides support information education and follow up to ensure continuity of care and to achieve optimal clinical outcomes and satisfaction. The Healthcare System Navigator is responsible for making recommendations and implementing changes as directed to program processes tools materials and supports and is a clinical and educational resource for physicians health professionals and patients in matters relating to total joint replacement surgery or other surgery/medical procedures.

Travel may be a requirement of this position. Transportation arrangements must meet the operational requirements of the Island Health in accordance with the service assignment and may require the use of a personal vehicle.

QUALIFICATIONS:

Education Training And Experience

Registration with the BC College of Nurses and Midwives as a practicing RN registrant. Post-basic education in surgical care nursing or an equivalent combination of education and training. Two (2) years recent related experience.

Basic Life Support Level C.

Valid BC Drivers Licence

  • Demonstrates ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning) and provide nursing care through evidence based guidelines
  • Demonstrates effective written electronic and verbal communication to facilitate inter-professional collaborative practice healthy environments and a respectful workplace for patients staff students and partners/affiliates
  • Demonstrates ability to apply knowledge of care planning and care coordination including knowledge of surgical pathways
  • Demonstrates ability to work independently and manage time/resources effectively
  • Demonstrates ability to perform the duties of the position.
