Schedule: 1:00 PM 9:00 PM and9:00 AM-5:00 PM 40 Hours per Week
Pay Rate: $55 $65 per hour

Job Summary:
Navitas Healthcare LLC is seeking a dedicated Licensed Master Social Worker (LMSW) to provide professional social work services including client assessments counseling and care coordination. The ideal candidate will support individuals and families through evidence-based practices to improve their well-being and ensure continuity of care.

Responsibilities:

Conduct psychosocial assessments and develop individualized care plans.
Provide counseling crisis intervention and support to clients and families.
Collaborate with interdisciplinary teams to ensure coordinated service delivery.
Maintain accurate and timely documentation in accordance with regulatory standards.
Participate in case conferences and contribute to treatment planning.

Qualifications:

Required Experience: Minimum of one 1 year of LMSW experience.
Required Education: Masters Degree in Social Work from an accredited institution.
Required Licensure: Valid Licensed Master of Social Work (LMSW) license issued by NYSED (Primary Source Verification required).

Preferred:

Experience working in healthcare community or behavioral health settings.
