The Lead QIDP-D / QIDP serves a blended role providing both direct service and program oversight within Hopewells Residential Department. This position is responsible for ensuring high-quality support to individuals with intellectual and developmental disabilities integrating both the practical daily-living skill development of a QIDP-D and the programmatic oversight and coordination responsibilities of a QIDP.
Approximately 70% of this role focuses on QIDP-D dutiesproviding direct support guidance and training to clients and staffand 30% on QIDP-level case management program development and service coordination.
DUTIES:
Direct Support and Program Implementation (QIDP-D 70%)
- Provides support and instruction in daily living activities including self-care domestic skills budgeting community integration health coordination and medication administration.
- Implements and documents individual program plans ensuring training and support align with established goals and objectives.
- Ensures a safe and sanitary living environment through regular home visits and monthly home inspections.
- Supports clients in applying for essential social service programsincluding housing SNAP Social Security vocational rehabilitation and other resources as needed
- Assists clients with budgeting reconciling checkbooks and paying bills on time.
- Provides on-call emergency assistance on a rotational basis.
- Transports clients using a personal vehicle to appointments errands and community activities.
- Completes all required documentation including service notes behavior data progress reports medication administration records and financial records.
- Provides ongoing communication and collaboration with the assigned QIDP to ensure consistent service delivery and client progress.
Program Coordination and Supervision (QIDP 30%)
- Oversees assigned client caseload ensuring health safety and training needs are met.
- Coordinates with healthcare professionals and external service providers ensuring effective communication and continuity of care.
- Supervises and supports assigned staff including hiring orientation training coaching providing performance feedback and disciplinary action and approving time worked and benefit time requests.
- Develops and reviews measurable training programs and individualized service plans.
- Monitors program data billing documentation client financial statements and compliance with state and agency standards.
- Conducts observations home visits and quality assurance reviews to ensure service excellence.
- Responds to emergency situations and provides guidance and necessary assistance to staff in crisis situations as needed.
- Maintains accurate documentation for case management progress reports and compliance reviews.
- Collaborates with team members to prepare for audits certifications and reviews (CARF FSSA Social Security etc.).
Leadership & Collaboration
- Trains and mentors new QIDP-D staff ensuring onboarding and ongoing competency.
- Provides coverage and assistance during extended absences of QIDPs or QIDP-Ds.
- Compiles and distributes community activity lists and assists with event coordination.
- Supports the creation and maintenance of emergency and weather plans for client homes.
- Promotes teamwork and open communication across the Supported Living team.
Qualifications :
- High school diploma or equivalent required; Bachelors degree in social work psychology or a related field preferred.
- At least one year of experience as a QIDP-D or in a similar setting required.
- Must obtain and maintain CPR and First Aid certification.
- Must successfully complete Core A and Core B Medication Administration Curriculum.
- Knowledge of the educational behavioral and daily living needs of adults with developmental disabilities.
- Strong understanding of individual program plans (IPPs) and ability to implement them effectively.
- Knowledge of budgeting community resources and social service systems.
- Ability to maintain accurate documentation communicate effectively and model professionalism.
- Must possess a valid drivers license and maintain a safe driving record.
- May be required to respond to emergency situations outside standard business hours.
