Summary
The Nurse Practitioner or Physician Assistantprovides specialty and preventive cardiac health care services to our heart failure collaboration with the attending physician would see patients in the hospital and outpatient setting. Coordinates specialty care and discharge management for acute care patients. As well as working closely with the Heart Wellness nurses cardiology providers primary care providers and Advanced Heart Failure and Transplant Cardiology colleagues from tertiary centers.
Education
For Nurse Practitioners: Completion of Masters degree in Advanced Practice Nursing or 4 years of formal training or education beyond high school (e.g. Bachelors Degree).
For Physician Assistants: Completion of an accredited Physicians Assistant Certification Program is required.
Certification Registration & Licensure
Certification required: ACLS.
Licensure required: Advanced Practice Registered Nurse: New Hampshire license as Advanced Practice Registered Nurse and DEA license.
Physician Assistant: New Hampshire license as Physician Assistant. DEA license for both APRN and PAs
Experience
Provider experience in heart failure is required
Responsibilities
Obtains patient health history and performs physical examination.
Orders and interprets diagnostic tests.
Diagnoses acute and chronic medical problems.
Prescribes and manages medications.
Provides preventive care secondary prevention and patient education according to practice standard.
Meets productivity standard of the practice/department.
Documents care provided in the medical record.
Maintains professional development.
