Overview
The Clinical Coordinator serves as a central team lead ensuring efficient comprehensive rehabilitation within a transdisciplinary model. The role focuses on supporting patients with brain spinal cord and other neurological injuries as they reclaim independence and reintegrate into the community.

Responsibilities
- Manage individual patient services and collaborate with the treatment team to develop and implement rehabilitation plans geared toward community reintegration.
- Formulate comprehensive transdisciplinary rehabilitation treatment plans clinical outcomes goals and service delivery for each patient.
- Complete initial progress and discharge reports and patient evaluations; monitor patient status and track clinical outcome tools based on diagnosis.
- Lead and facilitate interdisciplinary case conferences communicate treatment plans regularly with physicians payors and families and ensure thorough reporting.
- Serve as the main point of contact for patients families external case managers and physicians to coordinate progress and resolve issues.
- Facilitate acquisition of equipment and manage funding issues for ongoing rehabilitation goals.
- Negotiate with funding sources and insurance providers for extensions beyond initial service authorizations.
- Monitor projected service hours and report changes to site leadership.
- Participate in internal education staff development and continue refining clinical practice skills.
Qualifications
- Degree in Physical Therapy from an accredited program.
- Active Physical Therapist license in Nevada.
- Current CPR and Basic Life Support (BLS) certifications.
- Certified Brain Injury Specialist (CBIS) preferred.
- Minimum two years physical therapy experience in neurorehabilitation or closely related field.
- Prior clinical coordination case management or insurance administration experience preferred.
- Reliable transportation and ability to travel to multiple locations daily.
