Job Summary and Responsibilities
In keeping with Dignity Health philosophy the Medical Social Worker provides Social Service activities through collaboration with the care team and community partners coordinating and facilitating available resources to assist the caregiver and patient through the continuum of care.
Job Requirements
Required:
- Masters of Science (MSW) from accredited school of Social Work
- Minimum of 2 years experience in a clinical setting
Preferred:
- Experience in community referrals
- Licensed Clinical Social Worker: CA (LCSW:CA)
