Carolina Outreach is seeking a Forensic Assertive Community Treatment (FACT) Registered Nurse who is passionate about providing medical and psychiatric care to adults with serious mental illness and co-occurring substance use disorders who are involved in the criminal justice system. This role is ideal for someone who values teamwork community-based care and making a meaningful difference in the lives of others.
Build Your Purpose.
As a Forensic ACT Registered Nurse you will provide medication management health monitoring crisis intervention and care coordination for individuals involved in the criminal justice system. Serving Wake and Durham counties from the Raleigh office this position works collaboratively with law enforcement courts probation and parole officers and correctional healthcare providers to ensure continuity of care and promote successful community reintegration. The role focuses on forensic treatment planning risk assessment and coordinated care within the legal system to support each individuals recovery and long-term stability.
Position Highlights:
Administer and monitor medications provide health education and ensure adherence to prescribed treatments.
Conduct health assessments and screenings to identify physical and psychiatric needs requiring medical or clinical intervention.
Collaborate with criminal justice and behavioral health partners to coordinate medical and psychiatric care for justice-involved individuals.
Support reentry and continuity of care for individuals transitioning from incarceration to community-based services.
Participate in multidisciplinary team meetings providing medical input crisis support and documentation in accordance with regulatory standards.
