ðŸ Remote MA/LPN for Virtual Patient Care (Bilingual EN/SP Preferred)

Part-Time Flexible Hours US-Based Only

Lara Health is seeking a compassionate and experienced Medical Assistant (CMA) / Licensed Practical Nurse (LPN/LVN) to join our Chronic Care Management (CCM) / Remote Patient Monitoring (RPM) team. This is a fully remote role with flexible scheduling ideal for someone passionate about chronic care and patient engagement.

ðŸ¼ Position Highlights

Schedule: Minimum 20 hours/week
Location: Fully remote (U.S. based only)
Experience Required: Minimum 5 years caring for patients with chronic conditions (e.g. diabetes and HTN cardiology primary care)

ðŸº Role Responsibilities

Monitor patient vitals (blood pressure blood sugar etc.) and medication adherence
Support patients in scheduling MD visits and making lifestyle changes
Maintain accurate and up-to-date EMRs
Collaborate with providers and care team
Perform other duties as assigned

ðŸ Required Skills & Qualifications

Licensure: Active MA/LPN license (mandatory)
Experience: 5 years in clinical setting preferably in chronic care settings
Language: Bilingual English/Spanish strongly preferred
Strong communication organizational and time management skills
Ability to work independently and collaboratively
Proficiency with computers and EMR systems (must have own device and reliable internet)
Familiarity with medical terminology and clerical tasks
Passion for working with older adults and delivering preventative care

Familiarity with remote care coordination tools and EMR-based task management
Ability to delegate prioritize and oversee multiple patient cases simultaneously
