Your Opportunity

The Home Stretch WA model was co-designed with young people Aboriginal community members and the broader support system for those transitioning from Out-Of-Home Care. As one of the first services delivering Home Stretch in WA this is your opportunity to help shape a better future for young people moving toward independence.

As a Staying on Facilitator with Mission Australia you will play a vital role in supporting young people aged 1721 as they transition from Out-Of-Home Care to independent living. This is an on-site position and your work will be trauma-informed and culturally secure with a strong focus on self-determination and the wellbeing of young people their carers and carer households.

Your Key Responsibilities

  • Support young people and carers to develop Staying On Agreements that enable continued living arrangements until the young person turns 21.
  • Liaise with the Department of Communities to authorise and manage Staying On subsidies resolving any related issues.
  • Provide therapeutic support and guidance to carers including parenting strategies for young adults and conflict resolution.
  • Coordinate the review and support of Staying On Agreements in collaboration with Transition Coaches young people and carers.
  • Assist young people in accessing safe and stable housing including use of housing allowances and exploring options within natural networks and community housing.
  • Facilitate case conferences and family intervention meetings as needed.
  • Support family mapping reunification and contact with biological families.
  • Maintain accurate and comprehensive case management records and contribute to internal and external reporting.
  • Build and maintain strong relationships with stakeholders including the Department of Communities carers kin and community members.
  • Actively participate in interagency meetings Communities of Practice and professional development activities.

Requirementsfor Success

  • A Degree or Diploma in Social Welfare Social Science or a related field with at least 1 year of relevant experience child youth and family services.
  • Experience supporting young people in state care or those at risk of disengagement.
  • Demonstrated ability to work in a culturally safe and trauma-informed manner.
  • Proven skills in case management assessment and crisis intervention.
  • Strong understanding of Aboriginal culture and experience working with Aboriginal communities.
  • Excellent communication and interpersonal skills with the ability to build strong partnerships.
  • Knowledge of referral pathways and support services.
  • Full driver licence with no restrictions.

Before starting work with us you will need to undertake a national police history check qualifications and referee checks and have a clearance to work with children/vulnerable people.
