A panel may be formed as a result of this campaign for Falls Coordinator Older Persons Services for Dublin South & Wicklow IHA from which current and future permanent and specified purpose vacancies of full or part-time duration may be filled.
Purpose of the Post:
The Falls Service Coordinator will play a critical role in developing and integrating coordinated community-based falls prevention services primarily supporting public nursing homes. This capacity-building position will lead the implementation of evidence-informed changes ensuring that seamless multidisciplinary falls pathways are in place across the continuum of care.
The role will require:
Central Coordination: Establishing a single point of access for falls-related services encompassing multifactorial risk assessments prevention programs rehabilitation and community supports.
Integrated Care Pathways: Collaborating with acute and community services to design clear efficient referral assessment and onward referral processes.
Multidisciplinary Leadership: Convening and facilitating the local Falls Steering Group and relevant working groups to guide pathway development documentation training and dissemination.
Service Mapping and Data Management: Auditing existing services tracking referral patterns waiting times and outcomes. Identifying gaps in data collection and supporting adoption of the HSE Falls Prevention Framework (March 2005) recommended metrics and information systems.
Capacity Building: Leading phased implementation of multifactorial fall risk assessments in alignment with Integrated Healthcare Area (IHA) and regional service plans and the National HSE Falls Prevention Framework (March 2005).
Evidence-based Programme Delivery: Supporting the work of community exercise initiatives such as Falls Management Exercise (FaME) and Stay Steady groups consistent with World Falls Guidelines and the HSE Framework.
Continuous Quality Improvement: Embedding Quality Improvement (QI) methodologies including PDSA cycles and collaborative learning aligned to the HSE QI Framework to support sustainability and service enhancement.
