About the role:

This rewarding opportunity will see you join the St Vincents Private Hospital Toowoomba (SVPHT) as ourClinical Nurse Consultant - Discharge Coordinator. In this role you will ensure effective and timely admission/discharge processes are in place and to case manage patients with an extended length of stay and additionally to assist the Assistant Director of Clinical Services and contribute towards developing strategies to maximise cost-effectiveness and efficiency in the discharge process.

About the hospital:

St Vincents Private Hospital Toowoomba is the largest acute private hospital on the Darling Downs. We are a 197-bed facility with highly experienced and skilled staff that work as a team to deliver the highest standard of healthcare to all our patients. Our services include medical surgical maternity paediatric emergency cardiac and rehabilitation.

Things that work for us:

  • Registered with AHPRA (Australian Health Practitioner Regulation Agency) as a Registered Nurse

  • Ability to work on a permanent full-time basis (38 hours per week)

  • Previous experience in community and/or facility (or similar role)

  • Effective communication skills at all levels enabling you to achieve best practice

  • Consultative and liaison focused service aimed at supporting medical and nursing staff in the assessment care management and successful discharge of patients with necessary family and community organisations to ensure the smooth and safe transition out of hospital.

  • To ensure effective and timely admission/discharge processes are in place and to case manage patients with an extended length of stay

  • You will understand the importance of providing quality care and you feel rewarded by creating a genuine impact

  • Well-developed interpersonal skills that provide the support and nurture for our patients and organisations needs
