About the Role
We have an exciting opportunity for one part time Short Breaks Personalisation Officer to join our busy team!
The Short Breaks Personalisation Officers play a vital role in supporting families and disabled children/young people to access person centred appropriate short breaks provision.
The Personalisation Officers responsibilities can be split into two main areas:
access to Short Breaks Services for disabled children and young people in line with pathway one and two provision:
2. Assisting and enabling service users to use their Personal Budget to meet their social care and or health care needs by helping them plan and source both paid support and unpaid informal support from their social networks and communities.
About You
The ideal candidate will have high standards of communication with vulnerable adults and children as well as being able to work in partnership with other professionals and agencies and the ability to communicate in varied ways with disabled children and young people.
The Personalisation Officer is expected to work directly with citizens to identify appropriate support and provision in line with their personal support plan / care plan and direct payment (if relevant).
