Are you a caring and motivated Licensed Practical Nurse (LPN) ready to make a difference in the lives of others Were seeking a dedicated professional to join our dynamic healthcare team providing exceptional care to adult and geriatric patients.
What Youll Do
Deliver skilled nursing care under the supervision of an RN.
Collect and communicate patient data to support individualized care plans.
Assist in developing and revising care plans to ensure optimal patient outcomes.
Collaborate with physicians RNs and the interdisciplinary team to provide holistic patient-centered care.
Participate in mentoring and orientation for new staff.
Maintain knowledge and competency through continuing education and training.
What Were Looking For
Active NY LPN License (Required)
BLS Certification (Required)
Minimum 1 Year Experience as an LPN in a Long-Term Care (LTC) Facility
Compassionate dependable and team-oriented attitude
Ability to adapt to Day/Evening/Night/Weekend rotations and holiday coverage
