The Complex Care and Recovery Program is seeking a full-time permanent Team Lead for the Forensic Outpatient Service.Reporting to the Manager of the Forensic Outpatient Clinics the Team Lead supports the Manager to anticipate operational needs of the outpatient clinic. The Team Leader also works collaboratively with the Complex Care and Recovery Advanced Practice Clinical Leader Nurse Educator and interprofessional clinical team to ensure effective and safe coordination of client/patient-centered care consistent with CAMHs vision strategic directions and clinical practice standards. The Team Lead will assume responsibility for the communication liaison and coordination of client/patient admissions discharges transfers client assignments and tasks such as medication room audits. The successful candidate will facilitate service delivery ensuring a high standard of quality and safety. The team lead will act as a liaison and provide support to community partners and courtesy staff. The Team Lead will coordinate staff schedules to ensure appropriate coverage of the clinic lead daily/weekly team meetings attend treatment planning meetings as well as participate in quality improvement initiatives research and attend Provincial Community of Practice meetings. You will link new FOPS and courtesy staff with appropriate clinic resources and facilitate orientation to the clinic ensuring they become familiar with the clinics procedures/processes. You will coordinate and/or assign in collaboration with clinical supervisors/preceptors an appropriate CAMH staff to assist students of all disciplines educating them on team procedures and to assist with client/patient care issues. The Team Lead will collaborate with the manager regarding staff participation in off-unit meetings/ training educational events such as Nursing Practice Advisory Council and Education Rounds. You will support a healthy workplace that embraces diversity encourages teamwork and complies with all applicable standards and requirements and contribute to the development implementation and evaluation of best-practices all of which advance excellence in client/patient care. This position will be a regular day shift based at 1001 Queen Street West and may require weekends and travel for home visits.
The successful candidate must be registered with the College of Nurses of Ontario and be able to practice nursing without restrictions. A current B.C.L.S. (CPR) certificate is also required. Preference will be given to candidates who possess or are currently pursuing a BScN and CNA certification in Psychiatric/Mental Health Nursing. At least two (2) years (or 3900 hours) of clinical experience within the mental health and additions Outpatient field is required; with an applied knowledge of the forensic mental health system and recovery-based model of care. The successful candidate will possess proficiency and leadership in dynamic risk management principles case management responsibilities problem-solving decision-making conflict resolution negotiation communication delegation critical thinking team building and interpersonal skills. The successful candidate will possess the ability to think at a program-level and participate in unit-level as well as organization-wide change initiatives. The individual will have the ability to communicate effectively with the team including management both verbally and in writing; knowledge of the roles of interdisciplinary team members their professional responsibilities and areas of expertise is essential. Demonstrated expertise in maintaining a culture of safety for staff clients/patient families and visitors according to CAMH and regulatory standards is critical. Bilingualism (French/English) or proficiency in a second language would be an asset.
