Your Opportunity

Imagine being part of something truly transformative. Our brand-new service in the heart of Perth that is set to redefine how we support people experiencing homelessness Common Ground Perth has been developed with input from community housing organisations Aboriginal Community Controlled Organisations (ACCOs) and people with lived experience of homelessness and delivered in partnership with Nyoongar Outreach Services (NOS) this not just a building; its a bold step forward in creating long-term housing stability and holistic support for some of the most vulnerable members of our community.

This is an on-site position at a purpose-built facility in East Perth featuring 112 self-contained apartments many reserved for people who have experienced chronic homelessness. The site will also include communal spaces 24/7 support and integrated commercial areas all designed to foster a sense of safety dignity and belonging.

As a Housing Support Practitioner you will be at the forefront of this innovative model combining permanent affordable housing with tailored wraparound support services. From case management and healthcare access to pathways into education and employment you will help residents not only sustain their tenancies but also rebuild their lives and shape brighter futures.

Your Key Responsibilities

  • Deliver culturally safe trauma-informed holistic and person-centred case management underpinned by Housing First Principles.
  • Conduct participant intake and safety/risk assessments using established frameworks and respond to referrals from internal and external services.
  • Support participants with practical assistance to live independently including moving in to Common Ground budgeting passing property inspections and completing paperwork.
  • Facilitate and participate in group activities that build community capability and capacity to thrive at Common Ground.
  • Offer tailored information guidance and referrals to support individuals to access relevant community services.
  • Engage collaboratively with internal teams and external partners through case coordination advocacy efforts and connection to appropriate support networks.
  • Maintain accurate and timely case notes and administrative records ensuring compliance with reporting and quality standards.
  • Respond to trauma and crisis situations using Therapeutic Crisis Intervention (TCI) practices supporting participants with complex needs including chronic homelessness.

Requirementsfor Success

  • Minimum 2 years experience in case management within homelessness services with a relevant qualification in Social Welfare Social Science or Human Services.
  • Demonstrated experience in case management and working with people with complex support needs.
  • Strong knowledge and experience working respectfully with Aboriginal and Torres Strait Islander peoples and protocols.
  • Trauma-Informed Care (TCI) and a recovery-oriented practice approach is highly preferred.
  • Excellent communication decision-making and organisational skills with the ability to manage competing priorities.
  • Proven ability to follow policy and procedure contribute ideas and actively engage in service improvement.
  • Ability to work collaboratively across diverse teams and maintain professional boundaries in a fast-paced environment
  • First Aid Certificate is desired but not required.
  • Full driver licence with no restrictions

Before starting work with us you will need to undertake a national police history check qualifications and referee checks and have a clearance to work with children/vulnerable people.
