Job Summary

Compensation Range: $43/hr - $46/hr commensurate with experience.

Overview:

The Staff Nurse (RN) is responsible for the delivery of nursing care to clients and for participating in the development of interdisciplinary care plans.

Responsibilities:

  • Follows accepted standards of nursing practice by using sound scientific principles in providing direct care performing procedures and in using equipment.
  • Ensures that the nursing evaluation nursing care and recommendations for follow-up are documented dated and signed in the clients record in a timely manner.
  • Assesses clients within five visits in a time period not to exceed thirty days from registration and then on an ongoing basis.
  • Coordinates all necessary client services with other interdisciplinary team members in the comprehensive care planning process.
  • Maintains client caseload for the purpose of coordinating care both within the ADHC and with outside medical and case management providers.
  • Ensures the provision and coordination of client teaching regarding prevention transmission risk reduction and risk elimination strategies for HIV/AIDS.
  • Maintains accurate documentation of all nursing activities.
  • Coordinates clients medical care with external primary health care providers.
  • Assists clients in coordination of medical appointments.
  • Incorporates PCP/Clinic treatment orders into the programs treatment of the client whenever appropriate.
  • Reminds client of upcoming IDC/specialists appointments and obtain reports of findings.
  • Documents client visits to medical office in progress note section in eiCare/eCW.
  • Schedules all follow up visits.
  • Responsible for triaging client care.
  • Assess the physical factors relating to the clients illness need for care and response to treatment and adjustment to the program.
  • Address problems that may have an adverse effect on the clients ability to respond to treatment and ability to attain optimal functioning.
    • These problems may include but are not limited to dangers of polypharmacy review of Physicians instructions and management of personal hygiene and methods of optimization of ADL skills.
  • Conducts group with both clients and caregivers to provide education training and support related to the health needs of the clients and caregivers.
  • Provides nursing care to clients on a routine and crisis basis.
  • Participates in the formulation of nursing standards and in the evaluation of their effectiveness.
  • Participates in the appropriate Quality Assurance programs.
  • Reviews clients medical history as part of the initial evaluation and the 180 day reassessments.
  • Reviews and notes changes in the clients prescription on the medical sheet.
  • Reviews and assesses client compliance. Incorporates compliance and education issues into care plan if necessary.
  • Respond appropriately to emergency and acute medical issues and follows protocol to promote prompt attention to medical issues.
  • Verifies with PCP/Clinic that client is receiving proper prophylaxis for all opportunistic infections.
  • Acts as liaison to clients family significant others and personal attendants in the communication of health care information.
  • Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS.
  • Performs other duties as required by the Vice Presidents of ADHC Operations VP of Clinical Services and/or designee.
  • Must be a Registered Nurse licensed by the State of New York.
  • Bachelors degree from an accredited college or university.
  • Experience with persons with HIV/AIDS histories of substance use and/or mental illness a plus.
  • Experience working in a setting that promotes interdisciplinary care planning.
  • Bilingual preferred (Spanish/ English)
