The Community Translational Research Institute (CTRI) brings together key institutions and individuals from the public and private sectors including academic public health and community medicine institutions as well as neighborhood schools and community-based organizations for the prevention of chronic diseases. CTRI transforms population health practice through translation of prevention science into community-based programs and sustained practice and policy. The founding CTRI partners include leaders of the Claremont Graduate University School of Community and Global Health the County of Riverside the Inland Empire Health Plan and the University of California Riverside School of Medicine.

The Outreach Specialist will support outreach field-based engagement and enrollment efforts for the Enhanced Care Management (ECM) program connecting residents to coordinated health and social services. This role offers meaningful impact cross sector collaboration and hands-on experience in integrated care.
The pay rate for this role is $26.50 per hour.
ESSENTIAL FUNCTIONS
- Build trust with community members through culturally sensitive outreach and education.
- Organize outreach and enrollment events at local faith centers community hubs and public spaces
- Support Community Health Workers (CHWs) during home visits and follow up engagement
- Strengthen partnerships with churches nonprofits and local organizations.
- Track outreach activities and support program evaluation through basic data entry and reporting.
- Advocate for members access to health and social support systems
JOB QUALIFICATIONS
Education & Experience
- High school diploma or equivalent required; some college vocational training or relevant coursework in Public Health Social Work or Communications preferred.
- Bilingual English/Spanish fluency required.
- 1 years experience in outreach enrollment or community engagement roles.
- Demonstrated experience in community outreach engagement or event coordination.
- Active involvement or familiarity with Spanish Speaking community networks preferred.
Skills & Competencies
- Strong interpersonal and communication skills across diverse populations.
- Organized and detail oriented with ability to track and analyze outreach outcomes.
- Proficient in Microsoft Office Suite (Word Excel Teams Outlook Zoom)
- Comfortable using digital outreach and data tracking tools
- Collaborative team player with cultural humility and problem-solving mind-set
- Reliable transportation and valid drivers license required.
Schedule & Work Environment
- Part-time (20 hrs./week). Some early evening or weekend hours required for outreach events.
- Combination of office-based work field outreach and home visit support
- General office environment occasional local travel required.
