Cordial Health is seeking a dedicated and compassionate Psychiatry Nurse Practitioner to join our team. We are a leading healthcare organization providing comprehensive mental health services to patients in nursing homes across Trenton NJ. This is an excellent opportunity for a skilled PMHNP who is passionate about geriatric psychiatry and committed to improving the mental health and well-being of elderly individuals.
Responsibilities:
- Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes.
- Diagnose and develop individualized treatment plans based on the patients mental health needs.
- Provide medication management and adjust treatment plans as necessary.
- Collaborate with interdisciplinary teams including physicians psychologists and social workers to ensure holistic patient care.
- Offer counseling and psychotherapy services to patients and their families.
- Educate patients and their families about mental health conditions treatment options and self-care strategies.
- Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards.
- Participate in clinical meetings case conferences and quality improvement initiatives.

RVU: Compensation follows a structured RVU (Relative Value Unit) model designed to recognize clinical productivity quality of care and professional excellence.

- Proficient in electronic health record (EHR) systems and basic computer skills.
- Compassionate patient-centered approach to care.
