- Interview patients take vital signs and complete health screenings and referral forms
- Document patient observations vitals treatments and test results in the EHR
- Maintain a safe secure and confidential care environment
- Prepare patients for in-person or telemed appointments with medical prescribers
- Provide medication and home care education; answer patient questions
- Perform other duties as assigned by the center director or senior management
